type:text 目的：摂食嚥下機能が低下した在宅要介護高齢者（以下，療養者）と家族が経口摂取を希望する場合の多職種の支援の現状を明らかにする。そして，支援の現状を基に療養者と家族の経口摂取に向けた支援モデルを構築する。方法：まず研究１では，多職種の経口摂取への支援の現状を明らかにするために，多職種37名にインタビュー調査を行った。対象者の語りからカテゴリーを生成し，同様の目的を持つカテゴリーを統合したコアカテゴリーを要素として導き出した。次の研究２では，導き出した要素を用いて支援モデル案を作成した。さらに，支援モデル案を構成する要素の順序性，関連性，重要性を検討するために，専門家による会議を経て支援モデルを構築した。結果：多職種の支援として15カテゴリー（以下，【 】）が生成された。カテゴリーは【療養者の経口摂取の希望と生活の満足を受け止める】，【看護師は療養者と家族の希望する経口摂取に向けて多職種に働きかける】等であった。カテゴリーから生成した要素（以下，［ ］）は８つあり，［療養者の全身状態，摂食嚥下機能の情報収集と生活の全体像のアセスメント］，［療養者と家族と多職種での経口摂取と誤嚥に対する共通理解］等であった。考察：多職種は，療養者と家族の経口摂取への希望に応じるために，共通理解を得ながら進めていた。看護師は支援の中心的な役割を担っており，看護師のマネジメント力が重要であることが示唆された。 Purpose: This study aimed to establish a support model by interprofessionals that responds to the wishes of frail older adults with dysphagia and their families regarding oral intake, for that clarify the current state of interprofessionals support for frail older adults with dysphagia and their families. Method: This study comprises two parts. Study 1 involves the author interviewing 37 interprofessionals. Categories were integrated into core categories based on their similar purposes, and further led into elements. Study 2 involves the created of the original support model using the extracted elements. The appropriateness of the original support model was investigated by conference with experts. Finally, we established a support model. Results: We extracted 15 categories from the interview survey. Categories such as “interprofessionals catch the wishes of the frail older adults for food and their satisfaction with life,” and “nurses work toward interprofessionals to achieve preferred oral intake by patients and their families” were included. We extracted eight elements from the categories, “collecting information on the general condition and eating and swallowing functions of the frail older adults, so that assessing the overall lives of the frail older adults,” “common understanding of oral ingestion and aspiration among frail older adults, their families, and interprofessionals,” and so on. Discussion: Interprofessionals provided support in reaching a common understanding of the desired oral intake for frail older adults and their families. Nurses play a central role in providing support, and it was suggested that the management skills of nurses were important.
